Opinia lekarska:

Pacjent wymaga leczenia kierunkowanego i specjalistycznego, o które jest bardzo trudno w Polsce z powodu niewydolności systemu zdrowotnego.

[*The patient suffers from dyslexia and a few other language processing-related conditions.]

I've been thinking about "targeted treatment", but my confidence level is very low now.

The thing is 'kierunkowane' must not necessariliy be the same as 'ukierunkowane'.
As far as I understand, 'ukierunkowane' implies a predefined goal, while 'kierunkowane' may be directed on an ongoing basis.

Here:

and there is also the term 'czytanie kierunkowane' (=frontloading):

That being said, it is possible that 'targetted' (or something to that effect) might indeed be the right solution in this particular context.

In the case of radiation therapy, there is "leczenie Ukierunkowane," which indeed is targeted therapy, and there are several confirmations to that effect.

Frankly, I'm not quite sure myself, because 'directive therapy' is about specific therapeutical methods whereas your sentence: 'Pacjent wymaga leczenia kierunkowanego i specjalistycznego [...]' seems to refer to something more general (correct me if I'm wrong).
So, for my understanding, what may be meant here is 'individualized', 'guided' or 'targeted', still (coming back to the starting point).

Targeted therapy

Sugerowałbym użycie słowa "therapy"
