Bei diesem jungen Patienten war es zu einem Verschlussikterus gekommen und die weitergehende Diagnostik hatte den Verdacht auf einen Pankreastumor gestellt. Weiterführend war noch Angio-CT und PET-CT durchgeführt worden, im PET-CT ergab sich kein Hinweis auf Metastasierung und eine äußerst diffuse Anreicherung des gesamten Organs, so dass die Diagnose eines lokalen Tumorgeschehens zumindest in Frage gestellt werden musste. Es war mit dem Patienten und seinen Angehörigen die Probelaparotomie mit Probeentnahme und vor allem biliodigestive Anastomose bei dem progredienten Verschlussikterus besprochen worden. Im extremen Falle evtl. auch eine Pankreasresektion bis hin zur totalen Pankreatektomie. Nach ausführlicher Aufklärung und entsprechender präoperativer Vorbereitung führten wir o.g. Eingriff komplikationslos in Intubationsnarkose durch. Der postoperative Verlauf gestaltete sich im Wesentlichen unkompliziert. In der Zeit vom 21.04-23.04.2010 wurde Hr. x auf unserer Intensiv-Station überwacht und betreut. Am 23.04 konnte der Patient in hämodynamisch stabilem Zustand auf unsere periphere Station rückerlegt werden. Zum Verlegungszeitpunkt zeigt er eine ruhige Spontanatmung mit gutem pulmonalen Gasaustausch. Das Abdomen war weich mit nur geringem Druckschmerz und bestehender Peristaltik. Der Patient hatte noch nicht abgeführt. Die Extremitäten waren warm und rosig, der Verband war unauffällig. Es gab keine klinischen Hinweise auf eine Infektion. Auf unserer Station bestand eine gute Schmerzkontrolle, das Abdomen zeigte sich weich bei nur geringem Druckschmerz DK, PDK und ZVK wurden im Verlauf zeitgerecht entfernt. Der nach initialer Nahrungskarenz begonnene vorsichtige Kostaufbau bis auf Wunschkost wurde gut vertragen. Auf der peripheren Station wurde die Analgesie oralisiert und die parenterale Flüssigkeitssubstitution sukzessive ausgeschlichen.

In questo giovane paziente si era riscontrata la comparsa di ittero ostruttivo e gli esami diagnostici condotti successivamente hanno destato il sospetto di un tumore al pancreas. In seguito sono state eseguite un’angio TC e una PET TC; dalla PET TC non è risultata alcuna evidenza di metastasi ed un esteso interessamento dell’ intero organo al punto tale che la diagnosi di tumore localizzato doveva essere perlomeno messa in discussione. È stata discussa con il paziente e i suoi familiari l’eventualità di una laparotomia esplorativa con prelievo bioptico e soprattutto di un’anastomosi bilio-digestiva in presenza di ittero ostruttivo progressivo. In caso estremo, si sarebbe ricorso eventualmente ad una resezione pancreatica fino ad una totale pancreatectomia. Dopo un'indagine approfondita e un’adeguata preparazione preoperatoria abbiamo eseguito il sopracitato intervento senza complicanze in anestesia totale. Il decorso postoperatorio si è svolto sostanzialmente senza complicanze.
Nel periodo che va dal 21.04 al 23.04.2010 il signor X è stato tenuto in osservazione e trattato presso il nostro reparto di terapia intensiva. Il 23.04 il paziente è stato spostato nel nostro reparto di medicina generale in condizioni emodinamiche stabili.
Durante la degenza nel reparto il paziente ha mostrato una respirazione spontanea tranquilla con buoni scambi gassosi polmonari . L'addome era trattabile con lieve dolenzia alla palpazione e peristalsi valida. Il paziente non aveva ancora evacuato. Le estremità erano calde e rosee, il bendaggio era in condizioni discrete. Non c'è stata evidenza clinica di infezione.
Nel nostro reparto è stata riscontrata un’ottimale gestione del dolore, l'addome appariva trattabile con solo una lieve dolenzia alla palpazione, il catetere a permanenza, il catetere peridurale e il catetere venoso centrale sono stati tempestivamente rimossi. La cauta assunzione di cibo iniziata dopo il digiuno iniziale fino alla comparsa di fame è stata ben tollerata. Nel reparto di medicina generale la terapia del dolore è stata somministrata per via orale e l’assunzione per via parenterale dei liquidi è stata successivamente ridotta.
