Czech to English Medical Medical: Health Care Medical reports and recor

This is an acronym at the end of an outpatients record in the recommendation section for someone with genetic hemochromatosis (so I don't think it's related to bacteria in any way - which was all I could turn up in research. I have also searched linked to the clinic and hospital for any internally used acronyms, but cannot find anything I have confidence in).
