In extracorporeal membrane oxygenation (ECMO), blood is pumped outside of your body to a heart-lung machine that removes carbon dioxide and sends oxygen-filled blood back to tissues in the body. Blood flows from the right side of the heart to the membrane oxygenator in the heart-lung machine, and then is rewarmed and sent back to the body. This method allows the blood to "bypass" the heart and lungs, allowing these organs to rest and heal.

ECMO: hart-longmachine
ECMO staat voor extracorporele membraanoxygenatie. Bij sommige patiënten op Intensieve zorg is de long- of hartfunctie zo zwaar aangetast, dat een beademingsmachine en hartondersteunende medicatie onvoldoende ondersteuning biedt. Die aantasting kan het gevolg zijn van bv. ernstig hartfalen, ernstige bacteriële of virale longinfecties, longproblemen na een operatie of longontstekingen door verslikken.

De ECMO neemt dan de functie van de longen of het hart over. Het systeem wordt daarom ook hart-longmachine genoemd. Er zijn twee types, VA-ECMO en VV-ECMO, die worden ingezet afhankelijk van welk orgaan meer ondersteuning nodig heeft.

VA-ECMO
De veno-arteriële ECMO of VA-ECMO wordt gebruikt bij hartfalen met of zonder longfalen.

Het bloed stroomt dan uit een grote ader door een canule (holle buis) naar de kunstlong, waar zuurstof wordt toegediend. Het zuurstofrijke bloed komt vervolgens langs een andere canule terug in het lichaam, in een grote slagader. Zo neemt de ECMO de long- en de hartfunctie over. Dat geeft de organen de tijd om tot rust te komen en zich, met de hulp van medicatie, te herstellen.

VV-ECMO
De veno-veneuze ECMO of VV-ECMO wordt gebruikt als de longen niet in staat zijn om voldoende zuurstof op te nemen of koolstofdioxide uit te scheiden, ondanks maximale beademing.

Ook bij dit type ECMO wordt bloed uit een grote ader door een canule naar een kunstlong gestuurd, waar het van zuurstof wordt voorzien. Maar het bloed keert daarna door een andere canule terug in een grote ader. Het hart van de patiënt moet dus adequaat werken om het zuurstofrijke bloed door de longen en de rest van het lichaam te pompen. De longfunctie wordt zo overgenomen tot de longen voldoende hersteld zijn om zelf voor voldoende zuurstof te zorgen.
