If you look at the Google result here, https://www.google.com/search?q=cacfp child enrollment form ... ... then you'll see that the three results all include 'v ounce'. When you click on the links, you'll see that the PDFs actually don't say 'v ounce' but fractions of an ounce e.g. 1/4 ounce, 1/2 ounce.
