This is from a police report related to a car theft. I believe that "Vol + 5000 $ Ds/Sur" is an abbreviation of "Vol de plus de 5000 $ dans ou sur". I am not sure about "V.A." Some possibilities:
Véhicule automobile
Véhicule assuré
Voiture automobile
Voiture assurée

I have also seen "Vol + 5000 $ Ds/Sur V.R.". I do not know what "V.R." would mean in this context either.

I don't believe it includes the theft of the car as well as this is a separate charge (21351 Vol automobile)
