Tutti e due i termini derivano dall'inglese Briefing.
Nel primo caso, cioè imballaggi speciali, non standard, il cliente deve dare informazione/spiegazione; deve istruire l'agenzia.
Il modulo sembra quasi una specie di verbale di questo briefing; forse la tua traduzione è un pochino generica. Magari puoi usare il termine inglese, tipo "modulo/verbale del briefing sull'imbalaggio"
