Hi everyone,
I have come across this term in the employment regulations of a large company.
Among the documents that must be submitted to the company by those seeking employment, it is specified that the 履歴書 must have a 身分調書 attached. Is this simply a (copy of) an identity document? It seems, however, that 調書 usually indicates a record rather than a certificate.

I think "background check" or "details/proof/copy of background check" would work in this context. It's probably the term most commonly used in English for this kind of documentation.

: Known as the 'CRB check' or, more recently, 'DBS check' in the UK. Background check should do fine.

There can be various sections in a 履歴書 and the one that deals with oneself, giving details on oneself, is called 自分調書.

Personal record
I believe it is a statement about whether a person has a criminal record or has been declared bankrupt. I have suggested personal record, but I am not sure what we call this in English, or if there is any set term.

身分証明書 = ID papers (often abbreviated in 身分証 )
身分意識 = social status consciousness
身分制度 or jist 身分制 = class system
Therefore , being 調書 a written record/minute to give evidence of something enquired/investigated ...
I'd say, finally, that it is a request for substantiated personal information, but of social status (married/divorced etc.) and not a background check on criminal records.
Ovviamente il contesto completo puo' fare la differenza, per guidare una interpretazione piu' corretta delle sfumature di significato.
