"A quién va dirigido" means "a quién va dirigido el espontáneo". I don't think it's possible for "desde el espontáneo" to mean "spontaneously". "Espontáneo", contrasting with "sugerido", is a type of question. The difference is that the former is an open-ended question, such as "which brand do you like best?", without naming any for them to choose from, whereas the latter is a question like "which of the following brands do you like best?" Here's an example of a cosmetics survey in which the questions are marked "espontáneo" or "sugerido": "¿Qué marcas de Cremas faciales y corporales para el bebé conoces?"
