I think that if you say "Families" with disabilities, it is misleading because you might be saying that the whole family has a disability. If you use the term "family members" then you are saying that one or some of the people in a family have a disability.

: National organizations that promote the advancement of people with disabilities discourage the used of the word "disabled" because it focus on the disability and not the person. They use "people first" language, so People with Disabilities is the choice

I think I'd just leave "national" out of it, to avoid an overly long title. Alternatively, you could include "national' but remove "second annual". Yet another alternative would be "Second National Meeting...(omitting "annual).
"Persons with Disabilities" is the current politically correct term. "The disabled", "disabled persons" and "the handicapped" are to be avoided.

: I think that "persons" is a cold sounding albeit accepted word. I associate it with official forms and signs.
