This all goes to show how misleading poor handwriting can be, and how much misguided speculation it can produce. This is not to denigrate the contributions of Johnathan and Bourth which were both intelligent and thoughtful.

Can hardly be outpatient follow-up if this is the case.

given that this is a discharge notice "ambulatory follow-up care" would be logical. Still, there is no way that we can arrive at a definitive response without actually seeing the text.

I'd ask client if possible. If it's a case of reading handwriting, it's a must. In medical translation, especially, errors like that must be avoided.
