The wording "accessible to the disabled" is a commonly used phrase in the British NHS (google proves it!).
I believe "services accessible to the disabled" is enough and encompasses all on-site facilities as well as off-site services. For clarity though you could choose to include this, although I think Angebote is referring to the range of services in this context.
Referring to this on its own, I would say "accessible services" - this is also a term commonly used in the NHS (google shows this, too).

Just to explain for anyone unaware - NHS stands for National Health Service, i.e. the state-provided healthcare, which is the standard for the vast majority of British people.
I believe "services accessible to the disabled" is enough and encompasses all on-site facilities as well as off-site services. For clarity though you could choose to include this, although I think Angebote is referring to the range of services in this context.
