services (and facilities) accessible to the disabled / accessible services (and facilities)

The wording "accessible to the disabled" is a commonly used phrase in the British NHS (google proves it!).

I believe "services accessible to the disabled" is enough and encompasses all on-site facilities as well as off-site services. For clarity though you could choose to include this, although I think Angebote is referring to the range of services in this context.

Referring to this on its own, I would say "accessible services" - this is also a term commonly used in the NHS (google shows this, too).

Just to explain for anyone unaware - NHS stands for National Health Service, i.e. the state-provided healthcare, which is the standard for the vast majority of British people.

a broad spectrum of walk-in and other dedicated services

for starters
Peer comment(s):

neutral philgoddard : "Ambulant" means "outpatient", which is not the same as walk-in.

thx, meant to say "outpatient" as a matter of fact
neutral Harald Moelzer (medical-translator) : ...it definitely refers to services, but 'offen' means quite the opposite of 'dedicated'!!
