The latter is obviously referring to page progression.

Can one simply use "continuation" here, or is there some other technically relevant term in this context.

I agree that follow-up might apply, but that implies possibly more than is meant here. There is some small degree of difference between the 3 possible choices: continuation, follow-up, supplement. I think more context would be helpful

On many medical records I translate the one at the bottom of the page as ´continued´ and check that the ´page x of y´ is correct.

With the document in my hand, I can see that this is simply the usual, to make sure that all pages are present and in the right order.

I suspect that the ´Continuation nr. 26´ is simply a way of starting a new section.

Is it the first entry by a new consultant, or some kind of new ´chapter´ in the course of treatment?

Could it mean something like ´Continued from page 25´ ?

I think I have seen it used this way.
