This is from a physician's notes: It was decided to begin treatment with drug X, "a dosis de 2g cada 12 h3", with the 3 superscored. I understand "at a dose of 2 grams every 12 hours", but the 3 is something I've never run into. Does it mean three doses of 2 grams to be given 12 hrs apart? and how would this be abbreviated in English - "Q 12 h (3)", or what?

I also think it is an unusual way of saying "hourly".
If the context were an article for publication, the superscript could be a bibliographical reference, but as these are physician's notes, I don't think there is any other possibility except for "hourly".
