I understand what this means literally; however, it's used as a metaphor for something here and I can't figure out what for:

That's how I read it. Being born "ass-backwards" is the literal translation, but that implies stupid.

I’ve heard this used casually all my life, it could be common or just a regional quirk, (southern U.S., maybe?), but either way, it might be useful here.

this is an expresion we use in the United States to indicate that a person is unlucky in life.
Example sentence:
He is so unlucky he was born with both two left feet
