Hi all

This is in a document on the maternal and child health situation in Bolivia. The phrase occurs in this sentence, which is part of a section dealing on the need for skilled care during childbirth:

"Actualmente, persisten cuellos de botella en el acceso a los servicios de salud, tanto en la disponibilidad de los servicios como en el grado de importancia y las decisiones que les asignan las familias y las comunidades."

So, I think I grasp the idea - there are bottlenecks in accessing health services, both in terms of availability of services and in how they are viewed (the degree/level of importance) by families and communities (which thus affects the decisions/choices made about whether to access them).

What I can't get to is a concise structure to convey this idea - one verb doesn't seem to me to work in EN to cover both 'level of importance' and 'decisions'! All help in coming up with a solution would be much appreciated - and if anyone's reading of the structure is different, I'd be glad to hear that too.

Btw, I feel that "decisions/choices made about whether to access them" is a bit of an over translation, as, although this is very likely to be the case, the source text doesn't actually specify this, and it isn't a translator's job to embellish the source text!
