Hello, I am translating a document about Mexico's UN Millennium Development Goals program. The section in question in about improving obstetric care and care in general for pregnant women to lower the maternal death ratio. Several times, the term "hospital resolutivo" or "unidad (médica) resolutiva" appears. Here are some examples:


"Otro factor importante es la frecuente desarticulación a nivel de los estados entre el primer nivel de atención y los hospitales *resolutivos*, que son unidades que cuentan con infraestructura, equipamiento y recursos humanos suficientes para la atención obstétrica incluida la emergencia obstétrica, lo que conlleva la aparición de las tres demoras en la atención: el tiempo de traslado, el tiempo para recibir atención y la atención intrahospitalaria."


"En el primer nivel de atención, uno de los retos es fortalecer el inicio temprano de la atención de la mujer desde la etapa pregestacional, el control prenatal con intervenciones efectivas e integrales en la detección de riesgo y la referencia adecuada a las unidades médicas *resolutivas*."


"Aun con el sistema de salud y las parteras tradicionales, estas cifras evidencian lo heterogéneo de la distribución de personal por zonas, unidades y turnos de atención. En particular, el personal es insuficiente en las zonas alejadas, donde se envía a los médicos pasantes que carecen de competencias para la atención de calidad del embarazo, parto y puerpe rio y requieren de la referencia de las pacientes a centros de atención *resolutivos*."

“Para atender el compromiso de brindar calidad en la atención que permita un mejor resultado con el menor riesgo se requiere garantizar la capacidad resolutiva de la unidad médica, situación que no cubren todas las unidades médicas en operación, sino solamente aquellas que cuentan con especialistas, áreas clínicas y equipamiento adecuado para ello, a esto se le llama *‘resolutividad’*. Al amparo del convenio se deben cumplir los siguientes requisitos:

1.Unidad que cumpla con los criterios de resolutividad establecidos por la Comisión para la coordinación de acciones del convenio general de colaboración de emergencias obstétricas (nivel alto, medio y básico),

2. Acreditación de las unidades médicas conforme a lo establecido por la DGCES, y

3. Solicitud formal de la institución para su incorporación al Anexo 1, la cual se somete a la Comisión para su aprobación o rechazo.”

The secondary level can also be a referral level (from the primary level), but typically it refers to the third/tertiary level. So 'referral' is not the most precise option.

It comes up constantly in the translations I do for the Pan American Health Organization, where I worked for 30 years. Also, I lived in Mexico City for 2 years and am familiar with the term as it is used there
