I think IOP in this context stands for Inspección Ocular Policial, which means a visual examination of the crime scene by the police. The number will be that of the associated acta or record. Actually it's not just a visual examination; it includes processing the scene: collecting evidence, so personally I would omit "visual" in the translation. I think it just means as opposed to crime lab analysis.
This is from the Salvadoran "Manual Único de Investigación Interinstitucional":
"Equipo de IOP
Procesa el lugar de los hechos a fin de localizar, fijar y levantar las evidencias para el esclarecimiento del hecho; previa coordinación con la fiscalía. (Ver procedimiento: Inspección de la escena del delito.)." (p. 50)
"IOP: Inspección Ocular Policial." (p. 152)

As for RF, I think it's requerimiento fiscal. The following is from a thesis on criminal investigation by the Salvadoran National Police:
"REQUERIMIENTO FISCAL y DICTAMEN
R.F. INSTRUCCIÓN FORMAL CON DETENCIÓN PROVISIONAL:" (p. 282)
The details here show that it is an indictment. Note, by the way, that IOP is included among the "Diligencias iniciales de investigación" in the next column.
