payment plan? "Modelo" can mean "form" (as in a thing with data fields, e.g. application form) but it also has other meanings.
"Liquidación" is often translated as "settlement" but I feel it is more natural in English to say "payment" in most cases.
The problem term here — "modelo de liquidación" — may refer to how the cardholder pays off the debt, e.g. pay the lot at the end of the month, pay a percentage of the outstanding, pay a fixed amount per month.
