AND

This is exactly as it is typed in my text, with the dollar sign too (repeatedly, so I don't think it's a typo).

I think it stands for "partial" - just one part (1000ml) of the total urine output for that day; in fact the total (2500ml) is given afterwards.
The dollar sign is puzzling though - I've never seen that used on a ward.
