Contents. A ***patient's individual medical record*** identifies the patient and contains information regarding the patient's case history at a particular provider. The health record as well as any electronically stored variant of the traditional paper files contain proper identification of the patient.

In ordinary language I would call it an "information pack", but presumably that is not appropriate here.
The information pack might contain general information about what the patient is suffering from.

I can't see any reason to drop "information" in the translation. Sometimes, different countries simply have different procedures and tools and there is no clear equivalent in English. In such cases, a translation into English far removed from the source language is more misleading than a literal translation, at least when the literal translation is "clear". as in this case.
Also, in this type of document, the reader normally knows that it is a translation and so the goal is not to sound like the text was written in English in the first place but to shed some light on the alternative procedures and tools

: I'd say 'file', which covers both paper and electronic documents. We don't know if this is a patient record - it could also be drug information, or something else.
