Thx for the linguistic research you’ve done. I think ‘care advocate’ or ‘dedicated’ + [option A, B or C] is getting close to what this is, for it is not strictly residential support.

Background info: this type of social worker spends time with the client (a person with a significant mental handicap) at the institution where he/she spends the day. He/she might also pay some visits to the family to assess how the family dynamic of his/her client and give the caregivers some guidance/tips, though I can’t tell for sure based on the SLT alone.
