Custody warrant on extradition with consent

To: The officers of the authorised escort contractor and the Governor of HMP/YOI Wandsworth

Beign a person in respct of whom a Part 1 Warrant has been issued by a judicial authority of a category 1 territory, namely Czech Republic (hereinafer called the person) and having appeared today or been brought before me, the appropriate judge.

The person being legally represented has given his consent, before me, to his extradition to the category 1 territory in which the warrant was issued, which has been recorded in writing and is irrevocable.

Basis of remand: In proceedings under section 2(7) of the Extradition Act 2003
