I've got a Mexican hospital notes sheet in front of me. This is a heading for a paragraph of handwritten notes. So, I'll share with you the other headings:

Julie, I'm sorry I missed your question about medical acronym sources back in December. Now that I see it: COSNAUTAS is all you need (it's all I use) as an online resource. Cosnautas is by far the most comprehensive resource for medical acronyms, initialisms and abbreviations. It contains over 30,000 entries and gives many possible meanings for each (sometimes 30, 40, 50 or more depending on the term). See: www.cosnautas.com
Then we also have Ian Beattie's hard-copy masterwork, Medical Abbreviations Spanish-English (Abreviaturas Médicas Español-Inglés) with over 14,000 terms. He includes the English version of each (Cosnautas sometimes provides the English, but not always).

Not sure if it still works for you, but I found something that seems to fit the context. IMP stands for "Índice Maestro de Pacientes". It is part of the Health Matters National Basic Information System (Sistema Nacional de
Información Básica en Materia de Salud-SINBA) in Mexico. It keeps the records of an individual throughout their life including their contact with the health sector. Based on that, I think you could go for translating the acronym as "Patient Master Index" if you wished. You can check page 4 of the online PDF below for further details:

There might be clues to the meaning of IMP in the patient's notes. (It could be Instituto Mexicano de Psiquiatría, if that fits the patient's history.)
"history of present illness from the [IMP]" is how I would phrase that sentence
