Urkundlich/Zu Urkund dessen haben die Parteien zu dem oben zuerst genannten Zeitpunkt/Datum diese Vereinbarung getroffen.

or

Urkundlich/Zu Urkund dessen wird diese Vereinbarung von den Parteien zum oben zuerst genannten Zeitpunkt/Datum getroffen.

You will have to decide from the context how much flexibility you have to state this.
