I don't know what EDD means, perhaps E? Disability Determination. If the initials are conspicuous on the form, it would be better to keep them in English and offer the translation (above) in parenthesis.

Formulario Electrónico Estatal de Incapacidad
EDD means "Electronic Document Delivery" according to EUTERPE (European Parliament Terminology Database).
I am not familiar with this form, but it must be one concerning (physical or other/temporary or permanent) disabilities within a particular State which can be recorded on diskette or transmitted via e-mail.
Disability can be translated as incapacidad, discapacidad, invalidez or minusvalía. Although the latter is not PC language, it is however widely used in official forms in Spain.
The reference to "Eurodicautom" concerns only the term "disability", as I could not find EDD (except in EUTERPE).

Soy de México, es obvio que "state" al darle el tratamiento de Secretaría se refiere a regulaciones de gobierno, por lo tanto no es necesario, a menos de que en tu contexto se mencione el Estado XZ. En EU son Deptos Federales, en México son Secretarías. La información que se presenta en los formatos, complementa registros ya existentes. NO ASI EN LOS FORMULARIOS que formulan información no registrada aún en una base de datos.
