participant
"Langue propre, dentition complète. Abdomen symétrique, participant à la respiration, sans cicatrice opératoire visible, souple et sensible dans son ensemble..."
Not sure how to translate this. Is it that the patient is "actively breathing on his own"? Or somehow "participant" is referring here to the abdomen, as in "breathing (deeply including) from the abdomen."
Any suggestions?

I don't think the observation is connected to malaria, any more than is the observation about the patient's tongue and teeth and the absence of abdominal scars. However, advanced malaria can cause abdominal complications, so the comment souple et sensible might have some bearing.
Nor do I think participant à la respiration refers to abdominal breathing per se. Participant suggests a lower, secondary degree of respiratory involvement, maybe none at all, just an induced movement. As an asthmatic child I was a paradoxical-cum-thoracic breather and had sessions with a respiratory physiotherapist to learn belly breathing, so I know a little bit about such things.
That said, though I've been attempting to join the dots, this is really little more than a guess.

There are several ways of breathing, one involves the "active participation" of abdominal muscles.

From a functional point of view, there are three groups of respiratory muscles: the diaphragm, the rib cage muscles and the abdominal muscles.

As you can see, the action of breathing that you take for granted and are almost unaware of is quite complex with quite a few muscles at play. It is not surprising then that muscles in the abdominal region also play a role. Specifically, the rectus abdominis pulls the ribs down during active expiration.
