La technique à foyer ouvert consiste à inciser les tissus jusqu’à l’os, à réaliser la réduction grâce à des daviers, puis à fixer la fracture par un matériel d’ostéosynthèse. Ces techniques sont appliquées pour le traitement des fractures articulaires complexes. Certaines luxations irréductibles peuvent aussi être traitées à foyer ouvert.

Les techniques à foyer fermé sont actuellement plus souvent utilisées : la fracture est manipulée par des manœuvres externes à l’aide, pour les membres inférieurs, d’une table d’opération spéciale, appelée table orthopédique ou table de traction. Lorsque la fracture est réduite, on introduit le matériel d’ostéosynthèse par une petite voie. S’il s’agit d’un clou, celui-ci est introduit progressivement à travers le foyer. Cette technique a l’avantage de ne pas contrecarrer le processus de consolidation, d’apporter moins de risques d’infection et de ne laisser qu’une petite cicatrice.
