Explanation:
Macroscopic aspects of colonic cancer are represented by one of the following 3 types: vegetant tumors (almost always used for right colonic topography), ulcerative tumors (more frequently on the left colon, but sometimes associated with the vegetant form, due to the low intra-tumoral vascularization, leading to an ulcero-vegetant type) and infiltrative or stenosing tumor (almost specific for the left colon, with infiltration of the whole intestinal wall, on a limited distance - aspect of ligated bowel, respectively on a longer distance - aspect of rigid tube).

he peritoneum, like the pericardium and pleura, is a serous membrane that invests viscera. It is comprised of parietal and visceral peritoneum. There are many specializations of the peritoneum. All of the special structures that will be covered here are composed of two layers of peritoneum (much like the pulmonary ligament). They differ in location and what they connect. (Greek, peritonaion = stretch around)

Mesenteries: result from the invagination of "intraperitoneal" organs into the sac. The mesenteries connect viscera to the posterior abdominal wall and are VERY important in that they conduct blood vessels and nerves. (There are no vessels within the peritoneal cavity, of course.) The mesentery of the colon is usually called the "mesocolon". For example, we speak of the "transverse mesocolon" and the "sigmoid mesocolon". (The other parts of the colon are not completely invested by peritoneum, and are therefore "retroperitoneal".) Also, often "the mesentery" refers specifically to the mesentery of the small intestine. (Greek, mes = in the midst of, enteron = intestine)

Introduction: The distal resection margin (DRM) has been considered an important factor for the oncological outcome of rectal cancer surgery. However, the optimal distal margins required to achieve safe oncological outcome remains to be controversial.

Material and methods: More recently, as circumferential resection margin or mesorectal margin has been additionally reported to be more important factors predicting patient outcome than the distal mucosal margin, a re-evaluation of the impact of DRM on patient outcome is needed.

Results: The extent of distal tumor spread is known to be influenced by a variety of factors such as tumor location, lymph node metastasis, and tumor size. DRM might affect survival more than a local recurrence. Because distal intramural tumor spread rarely exceeds 1 to 2 cm in most rectal cancers, and local control and survival do not seem to be compromised by shorter distal resection margins, the generally accepted practice is to aim for a 2-cm DRM. However, in the recent trend of curative resection after preoperative chemoradiotherapy, with an otherwise favorable tumor such as well-differentiated tumor and no lymph node metastasis, a DRM at < or =1 cm does not necessarily portend a poor prognosis. In cases with preoperative chemoradiotherapy, distal resection margins need to be evaluated individually.

What are the segments of the colon?
The entry point, about six inches long, is called the cecum. The rest of the colon is divided into segments: the ascending colon (traveling up), the transverse colon (traveling across to the left), the descending colon (traveling down) and the sigmoid colon (headed back across to the right).

Colorectal cancer (CRC) - 8 cm long colon segment is upstream/traveling up on the peritoneal side of a 2/1 cm ulcero-vegetant type tumor formation located 1.8 cm from the distal resection margin (DRM) and 4 cm from the proximal resection margin (PRM).
