I think this means that the appeal against a ruling by the tax administration went straight to appeals court, no court of first instance or trial court involved.
Is there an elegant way to say this in English?
The client is in the EU, so British usage is fine.

and "in grado unico" is "at sole instance" (see, for example on this page: http://eur-lex.europa.eu/legal-content/EN/ALL/?uri=CELEX:619...

Therefore, this would seem to be an acceptable translation, even though I admit that the expression "proceedings at sole instance" is not one I have seen before

I think you are on the right track. The appellant jumps to the appellate court to challenge the Bank of Italy's penalties without going through a trial court. Again, the problem is using a concise expression meaningful to an English speaker.
