I agree with Adrian MM in that this has nothing to do with a written summons. I would venture to say that it probably refers to the type of report that a police office draws up when a person files a complaint in person at the police station or before an administrative authority. However, without context regarding what the report actually contains, I can't give a higher confidence level.

Yes, 'claim'/ 'complaint' both valid in my opinion too;
disagree Adrian MM. : It's not a written summons, rather the opposite, so of pressing charges in person & taken down by a police officer or magistrate. See Phil G's second weblink // Comment: maybe you also mixed up accusation with acusación as a a serious-crimes indictment.
